Re:	Annual Offence Declaration
	In order to maintain the currency of the Vulnerable Sector Screening Check (VSS) that The Royal Conservatory (RCM) has on file for you, you must complete this Annual Offence Declaration Form on an annual basis. The Royal Conservatory reserves the right to request an updated Vulnerable Sector Screening Check at any time throughout your employment. It is your obligation to immediately inform The Royal Conservatory of any events that would alter your Vulnerable Sector Screening Check. I DECLARE THAT:	Since the last Vulnerable Sector Screening Check collected by The Royal Conservatory, up to and including the date of this declaration, I have no convictions under the Criminal Code of Canada, or any comparable criminal legislation in any other jurisdiction or convictions for which a pardon has not been issued or granted under the Criminal Records Act (Canada), or any comparable criminal legislation in any other jurisdiction. Since the last Vulnerable Sector Screening Check collected by The Royal Conservatory, up to and including the date of this declaration, I have convictions under the Criminal Code of Canada, or any comparable criminal legislation in any other jurisdiction or convictions for which a pardon has not been issued or granted under the Criminal Records Act (Canada), or any comparable criminal legislation in any other jurisdiction.
